What are the conditions or factors that contribute for a company not to be able to claim VAT?

Panwa :  A company would not be able to claim VAT if:

Don’t have the original tax invoice such as the company did not issue the tax invoice or issue but input the other person.
The tax invoice was lost and can’t  find it
The contents of the tax invoice has error or has incomplete important information
Input vat by the tax invoice issued by the one who can’t issue the tax invoice.
